Forms and documentation required by Mitsubishi Motors North America: Proof of active duty, active reserve, retired or veteran U.S. Military Service. Active duty or active reserve personnel must provide one of the following documents: (1) a current (within the last 30 days) Leave and Earnings Statement (LES); (2) Department of Defense (DOD) letter from their Commanding Officer verifying active duty status; or (3) print out from https://scra.dmdc.osd.mil/scra/#/single-record verifying current active duty status. Retired U.S. Military personnel must provide one of the following documents: (1) current (within the last 12 month) Retiree Account Statement (RAS); or (2) most recent 1099-R or CRSC (Combat Related Special Compensation); or CRDP (Concurrent Retirement Disability Pay) Statement from Defense Finance and Accounting Service, U.S. Military Retirement Pay. Grey-Area Retirees who do not have RAS must provide both of the following documents: (1) DOD letter notifying Eligibility for Retired Pay at Age 60 and (2) current driver’s license. Grey-Area Retirees who are over 60 years old must also provide copy of current (within the last 12 month) RAS. Veteran who is within 12 months of separation from active service must provide DD Form 214, Certificate of Release or Discharge from Active Duty. Spouse of eligible U.S. Military personnel when only the spouse is listed on the purchase/lease agreement: must provide proof of U.S. Military Status described above and copy of marriage certificate. Surviving spouse of eligible U.S. Military personnel: must provide Proof of U.S. Military Status described above or DD Form 214, Certificate of Release or Discharge from Active Duty and Marriage certificate of the U.S. Military personnel. Private information such as your SSN, driver license number, or birthdate must be redacted from all submitted documents before they can be accepted.”
